Within 7 days, funds — UAH 2,000 — will be credited to the card. They can only be used to undergo the Health Screening 40+ at a designated list of facilities.◉ Choose a facility and sign up: on the official MOH landing page (screening.moz.gov.ua), choose the nearest medical facility (for example, the Primary Healthcare Center of Boyarka City Council), and book a convenient time for your visit.◉ Come for your screening and pay with the program card.✌🏻 Option 2: Through the Administrative Services Center (CNAP), if you don't have Diia◉ Order a plastic card with a special account at designated banks.◉ Visit any CNAP and submit an application to participate in the Health Screening 40+ program after 30 days from your birthday.◉ After approximately 7 days, expect an email notification about the crediting of funds.◉ Choose a medical facility on the official MOH landing page ➜ screening.moz.gov.ua (for example, the Primary Healthcare Center of Boyarka City Council) and sign up for screening.
